The Drive Poly-Fly Transformer Transport Wheelchair is a lightweight wheelchair that easily converts from a self propelled, lightweight wheelchair to an assisted transport wheelchair in a snap, using quick release axles to remove the rear wheels. Folds compactly and is easy to handle.

With the simple push of a button your patient or care giver can remove the 24" rear wheels and convert to a lightweight transport chair.

Features an all aluminum alloy frame, dual wheel locks for either version, flip back arms, fold down back, 24" rear wheels (removable with quick release axles), four 8" x 1" front and rear casters, footrests, and easy to clean black nylon upholstery.

Weight 33 lbs (TR18) when used as a wheel chair (without riggings) and 22 lbs in transport mode.

The Medi-Planner organizes medication for an entire week and accommodates dosage schedule for those needing medication up to four times a day. The easy-opening lids are labeled in Braille and large lettering for those with visual impairments. In total, there are 28 compartments.

Provides convenience of a disposable inner cannula for a wide variety of applications. Disposable design minimizes risk of cross-contamination and helps protect healthcare workers. Disposable Inner Cannula (DIC) features integral 15mm snap-lock connector for quick, safe disposal after single use. Smooth, rounded-tip obturator facilitates insertion. Disposable Decannulation Plug (DDCP) closes the proximal end of the DCFS and DCFN to permit breathing through fenestration and upper airway. Designed for use to bypass upper airway obstructions, provide long term ventilation, support and/or manage tracheal/bronchial secretions.

Picture dialer phone is ideal for fast dialing identification. Insert up to ten 1" x 1 1/2" pictures to the keypad. Program ten one-touch or 10 two-touch memory numbers. Features a switch that allows for one button picture dialing only. This feature is ideal for individuals who have difficulty or cannot dial numbers. All programmed numbers are backed up and safe even if unit is unplugged from the line.

Phone can be switched to dial by picture only. ( Ideal for loved ones with Alzheimer's or Dimentia)
